UHCCA respects your privacy and your right to limit information exchanges to only those you initiate.

In response to this survey you may have provided personal information. If so, the personal information you provided will be used solely to send you communications from UHCCA from time to time.  UHCCA will not voluntarily disclose a private individual’s personal information to any third parties for marketing or promotional purposes, or for any other purpose unless a court orders us to do so. We have put in place appropriate procedures to safeguard and secure the informa­tion we collect to prevent unauthorized access or disclosure, maintain data accuracy, and to ensure the appropriate use of information. If, despite our security precautions, personal information about a private individual is inadvertently disclosed, we will do everything reasonable to limit and remedy the disclosure.
